Documentation Needed At Time of Registration

• Completed Registration Form
• $125 Non-refundable Registration Fee
• Birth certificate
• Baptismal Certificate, if child was baptized
• For Students entering grades 2 through 8: a copy of latest report card from school from which the child is transferring

Age Qualifications for Early Childhood and Kindergarten

• Nursery School - must be 3 years old by December 31st of the school year starting in September and completely toilet trained.
• Pre-Kindergarten - must be 4 years old by December 31st of the school year starting in September and completely toilet trained.
• Kindergarten - must be 5 years old by December 31st of the school year starting in September.

Other Information/Forms Required

• Medical Forms completed and signed by Physician for children entering Nursery, Pre-K or Kindergarten.
• Tuition Preference Form / FACTS Payment registration Form
